Personal Income Tax Document List:

➤ S.I.N. (Social Insurance Number)

➤ T4, T4A, T5, T3… (all taxable slips)

➤ Name, address and Date of Birth (the same for the family if it applies)

➤ Medical expenses: Dentist, medical exams, pharmacy receipts (only prescriptions)

➤ Donations receipts

➤ Union receipts

➤ RRSP receipts

➤ Rent receipts (with landlord’s name, amount paid and how many months lived in the address for the year) or

➤ Property Tax paid for the year

➤ Home Buyer’s Plan – Withdrawal Amount and date and Repayment Annual Amount

➤ If bought a house and is a First Time Buyer: please provide complete address and date of purchase

➤ If sold a house: Date sold, Amount sold, Year of purchase, Amount paid on acquisition and if it was your Principal Residence or not

➤ List of Properties/Investments outside Canada if more than $100K in total

➤ School/Courses/Tuition Fees receipts if each receipt is over $100

➤ Child Care expenses

➤ Disability Amount Credit. If yes, please provide First Year and Last Year of eligibility

➤ Interest earned on investments and Interest paid for investments loans in the year

➤ If you moved to Canada in the year of the tax return you’re filing please provide the date of entry and total income made until the date you moved to Canada

➤ If you moved to/from a Province in Canada in the year of the tax return you’re filing please provide the moving date.

➤ Expenses:

⎈ car repairs, gas, insurance, plates renewal, parking, 407 ETR, lease payments, advertising, telephone, promotions, donations, wages employees/helpers/subcontractors, meals & entertainment, business fees/licences, delivery/freight and express, office expenses, legal and accounting, rentals and all expeses that are related to the business

⎈ Plus personal expenses:
